New patient paperwork must be returned at least 24 hours prior to your initial visit.

Payment is expected at the time of service. We accept cash, personal checks, debit or credit cards from Visa, MasterCard, Discover and American Express. We also accept Care Credit Checks returned for any reason will incur a $30 fee.

Flexible spending accounts and Health Savings Accounts (HSA’s) are also accepted for payment.
